👶 Date of Birth
Select your birth date from the calendar. You must be at least 18 years old.
🚻 Gender
Choose your gender. This helps us personalize your Health Score.
📏 Waist (in inches)
Measure around your belly button (not the narrowest part of your waist). Enter that number in inches.
📐 Height
Enter your height in feet and inches. Example: If you’re 5'8", type “5” in the feet field and “8” in the inches field.
⚖️ Weight (in pounds)
Type in your most recent body weight in pounds.
🩺 Blood Pressure
Use your latest blood pressure reading. You can find it on a home monitor, recent doctor visit, or pharmacy machine:
- Systolic – the top number (when your heart beats)
- Diastolic – the bottom number (when your heart rests)
🧪 A1C (%)
Required: This test measures your average blood sugar over the past 3 months.
You’ll find your A1C on lab reports under names like:
- Hemoglobin A1C
- HbA1c
- Glycohemoglobin
It is often listed under “Diabetes Panel,” “Glucose Panel,” or “Metabolic Panel.” Make sure it’s from within the last 3 months.
💪 HDL (mg/dL)
Required: HDL is your “good cholesterol.”
Look for it on a lab report called “Lipid Panel,” “Cholesterol Test,” or “Lipid Profile.”
It’s usually labeled as HDL or HDL-C.
🩸 Triglycerides (mg/dL)
Required: This is another value found in your cholesterol or lipid panel.
Look for a number labeled Triglycerides in your lab results, usually grouped with Total Cholesterol, HDL, and LDL.
